Hospice Toronto works on a care team model, with the belief that teams of caregivers from different walks of life can best meet the needs of someone who chooses to die at home. A care team often includes spouses or partners, families, friends, neighbours, colleagues, doctors, nurses, personal support workers and volunteers. They can help set up the team, provide team members with training and support, and supplement the team with skilled volunteers.

Mission

Hospice Toronto's mission is to enable access to appropriate care and support for those with serious illness and their caregivers — when they need it, where they need it, and whoever they are. The organization fulfills this purpose by providing continuity of care through all phases of a client's illness, educating clients and the public about choices around dying at home, ensuring services are welcoming and accessible to Toronto's diverse communities, and advocating for excellence in client care at all levels of service and social policy.

Hospice Toronto envisions a future in which every person living with a serious or life-limiting illness, and their family caregivers, can choose to receive compassionate, high-quality care at home — including the choice to die at home — with the support they need, when and where they need it. The organization works toward a healthcare landscape shaped by innovation, imaginative partnerships, and community resilience, where no one faces the end of life alone and all communities, regardless of background or means, have equitable access to palliative care.

Unique value proposition

As Toronto's first volunteer- and community-based in-home hospice palliative care program, Hospice Toronto holds a distinctive and pioneering place in the city's palliative care landscape. All of its services are provided entirely free of charge, and its care-team model — in which rigorously screened volunteers receive nearly 40 hours of training to deliver both practical and emotional support in clients' homes — sets a high standard for volunteer-driven hospice care. The organization is embedded in Ontario's broader health system through participation in three Ontario Health Teams and the Integrated Client Care Program, enabling seamless collaboration with nurses, physicians, and personal support workers. Its scope extends well beyond end-of-life care to include dedicated programming for young carers and a nationally recognized community development model that has been scaled to Nova Scotia and other parts of Ontario with federal support, reflecting a commitment to innovation, partnership, and equitable access across diverse communities.

WellFunded charges philanthropists today. For charities, WellFunded offers a free tier, with plans for affordable paid options that provide enhanced visibility and engagement tools. There will always be a free tier.

Instead of relying on cold proposals or personal networks, WellFunded helps you get in front of philanthropists actively looking to give in your area of impact.

How does WellFunded ensure trust and transparency?
